A physician evaluates and determines eligibility for home health care.
Home health services are ordered by the physician based on the patients’ needs.
When needed a care coordinator meets with the patient bedside.
A clinician provides in-home visits based on the physician’s orders.
A representative follows up with the patient to review progress and assess any additional needs.
Home Hospice Care Process
A physician evaluates and determines eligibility for home hospice care.
Home hospice services are ordered by the physician based on the patients’ needs.
A care coordinator meets with the patient and family in person or by phone to help coordinate care.
Our nursing team completes a full patient assessment to begin hospice care and arrange initial visits.
The hospice care team schedules visits according to the patient’s care plan and individual needs.
